Description
The cardiac nurse navigator serves as a consistent resource to support optimal care delivery for coronary artery bypass graft (CABG) patients and other projects as assigned, focusing on coordination of care during pre-procedure, hospitalization, discharge planning and facilitating post-hospitalization follow-up. The nurse navigator is knowledgeable of multiple treatment decisions, cardiac evidence-based standards, best practice guidelines and maintains care collaboratively across the continuum.
In collaboration with clinicians, identifies and facilitates optimum cardiac care delivery with clinical expertise, strong interpersonal abilities and critical thinking skills. Provides effective oral or written feedback to providers to identify care gaps.

Responsibilities- Coordinate care for CABG patients across pre-procedure, hospitalization, discharge planning, and post-hospitalization follow-up.
- Collaborate with clinicians to identify and facilitate optimum cardiac care delivery using clinical expertise and critical thinking.
- Provide effective oral or written feedback to providers to identify and address care gaps.
- Bachelor’s Degree in Nursing from an accredited school, with a focus on cardiac surgery inpatient care.
- Washington Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 3 years of experience as a Registered Nurse in cardiac surgery.
- 5 years of experience as a Registered Nurse in cardiac surgery.
