License: Must have license in hand

Certifications: CPR, BLS

• Proficiency in OASIS and Home Care Home Base EMR
• Reliable transportation
• Current driver's license
• Agency-required liability insurance
• Will be floating to nearby clinic to pick up additional hours if needed

• Coordinate and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits and attend periodic office meetings.
• Manage a patient load of approximately 5-8 patients per day.
• Responsibility for documentation and travel activities, with productivity expectations of 30 points per week.
• Handle a variety of cases, including Start of Care, Wound Care, and Follow-up Visits.
• Comfortable with on-call rotation and traveling up to 75 miles from the home clinic.
• Comply with the requirement of completing all documentation within 24 hours of the patient appointment.
• Participate in weekly meetings at the clinic.
• Be proficient in using Home Care Home Base EMR, and willing to complete a retraining course if needed.

Note:

The contractor will not have guaranteed hours and must achieve weekly productivity points equivalent to a 40-hour workweek.
