** Care Transitions Service Line.
** The ultimate goal of this role is to ensure that we are providing the right care in the right place at the right time...every time.
This service line brings together the Social Work, Case Management and Utilization Review functions under a single director. It also expands our Physician Advisor role.
If selected for this exciting role, you'll report directly to the Chief Nursing Officer with a dotted line relationship to the CFO. To be successful, you'll need proven change management and process improvement skills plus a strong understanding of hospital operations.

* Qualifications:
*** At least 5 years of healthcare management experience in an acute care setting.
* At least 3 years of recent case management, discharge planning or utilization review experience.
* Bachelor's degree. (Master's degree preferred.)
* Current RN license in Washington state or a compact state.
