• Conduct comprehensive resident assessments prior to move‑in, re‑entry, and on an ongoing basis to ensure all physical, mental, and social needs are met.
• Assist with community readiness for regulatory inspections, including maintaining accurate service plans, medication records, and emergency preparedness.
• Establish and foster positive relationships with residents, families, associates, home health, hospice, and other external partners.
• Coordinate and ensure proper health‑related training and education for the caregiver team, conducting skill observations to optimize resident care delivery.
• Provide direct resident assistance as needed, including personal care activities and administering medication in accordance with physician orders.
