The Neighbourhood Clinic is affiliated with the Belleville and Quinte West Community Health Centre (BQWCHC) but willoperatein a separate location providing Primary Care services to better serve our community. The Clinic is part of the HPE OHT’s strategy to increase access and attachment to primary health carein HPE.
OpportunityAs a key member of the interdisciplinary health care team, the Registered Nurse provides comprehensive clinical care and coordination within a general clinic setting, working to full scope of practice with an emphasis on extended and preventative care across the lifespan. The RN practices with a high degree of clinical autonomy and collaborates closely with physicians, nurse practitioners, and other allied health professionals to support advanced assessment, care planning, treatment implementation, follow-up, and evaluation of outcomes.
The role has a strong focus on chronic disease and prevention management, including ongoing care for clients with conditions such as diabetes,COPD, asthma,CADand other long-term health conditions,utilizing evidence-based guidelines to support self-management, monitoring, and early intervention. The RN also delivers population-based preventative health initiatives, including immunizations, screening programs, health education, and lifestyle risk reduction.
The RN plays an integral role in health promotion, client education, screening, and continuity of care, supporting improved long-term health outcomes through proactive, evidence-based practice.
- Current registration in good standing with the College of Nurses of Ontario and Professional Liability Protection
- Three plus years nursing experience in a primary care setting or combination of primary care and community/hospital/public health setting;demonstratedability to work under Medical Directives.
- Experience oradditionalcertification in primary care areas an asset (pediatric assessment, women’s health,smokingcessationand preventative care) an asset
- Certification or willingness to pursue CDE (Certified Diabetes Educator) and/orCRE (Certified Respiratory Educator)
- Certification or willingness to pursue RN prescribing
- Clinical leadership experience in chronic disease management is an asset
- Experience with electronic health care records; experience using PS Suites anasset.
- Excellent problem-solving,organizational,and interpersonal skills with an ability to work effectively and efficiently inafast-paced,collaborative, interdisciplinary team environment.

- This role includes primarily daytime clinic hours, with some evenings, to better accommodate patients’ needs and improve access to care.
