• The Transfer Center Clinical Case Manager coordinates the transfer of patients from, to, and between Harris Health facilities.
• Functions independently in the prioritization and verification of transfer requests.
• Maintains department records and statistics associated with transfer requests.

• Degrees:
  Associate Degree or diploma required; BSN preferred.
• License & Certification:
  Current valid Registered Nurse License in the State of Texas. Basic Life Support Certification required.
• Work experience:
  
  Five (5) years experience as a Registered Nurse. 1-2 years experience with Case Management, Quality Management, Coding or Discharge Planning preferred.
• Management
  
  Experience:
  
  Two (2) years management/supervisory experience preferred.
• Communication
  
  Skills:
  
  Above Average Verbal (Heavy Public Contact);
  Writing / Reports.
• Proficiencies: MS Word, PC.
• Knowledge/Skills/Abilities:
  Analytical, Medical Terms.

• Equipment Operated:
  Basic level computer application experience/skill (Excel, Word, etc.).
