Job Summary
The Advanced Practice Nurse, Hospital at Home (APN-HaH), is responsible for providing comprehensive patient care within the designated population (Family/Individuals across lifespan and Adult/Gerontology) through assessment, diagnosis, treatment, management, and follow-up, under the supervision of the HaH Clinical Medical Director. The APN-HaH serves as a clinical expert and consultant to support patient-centered, culturally competent, outcome-focused therapeutic care within the management of specified populations, through independent and collaborative clinical management.
The APN-HaH ensures practice aligns with professional rules, regulations, accreditation standards, and organizational policies.
Clinical Summary
This APN-HaH provides high-quality patient care within the Hospital at Home Program model. Responsibilities include planning, implementing, evaluating, and communicating all phases of the patient's care plan to the medical and nursing teams. The APN is responsible for fostering an environment that supports professional practice and empowers interdisciplinary healthcare workers to deliver safe, effective, compassionate, and efficient care.
Minimum Qualifications
Degrees:
- Bachelor's Degree
- Master's Degree in Nursing from an accredited school, with completion of a university-based program for Advanced Practice Nurse;
Acute Care NP or Adult-Gerontology Acute Care NP preferred
Licenses & Certification:
- Current specialty certification from recognized certifying organizations
- Current prescriptive authority (or eligibility) from the Texas Board of Nursing
- ANCC Certification
- DEA registration (or eligibility to obtain within 3 months of hire)
- Recognition as APN with the Texas Board of Nursing
- Current RN license with the Texas Board of Nursing
- Current Basic Life Support certification
- Current Advanced Cardiac Life Support certification (as appropriate)
- Valid Texas Driver's License
Work Experience:
- Minimum 5 years of experience, preferably as a hospitalist
Management
Experience:
- At least 2 years in acute care settings such as emergency or inpatient
Language
Skills:
- Bilingual in Spanish preferred
Communication
Skills:
- Strong verbal communication, including public speaking
- Proficiency in writing, reports, and correspondence
Proficiencies:
- MS Word
- Personal Computer
Job Attributes
Knowledge, Skills, and Abilities include analytical skills, mathematics, medical terminology, design, research, and statistics. The work schedule is flexible, including weekends, holidays, travel, on-call duties, telecommuting, and overtime. Other requirements include advanced clinical knowledge, conflict resolution, communication coaching skills, and functioning motor vehicle operation. Equipment operated includes tablets, laptops, and portable devices.
