Behavioral Health Case Manager- Children and Adults

$50-60,000 possible, depending on case load

We are looking for eager, compassionate case managers to join our team! As we grow and continue to build new partnerships in the community we are adding to staff and seek 2 case managers who is willing to assist and support children, adults and families in Knox and Waldo Counties. One case manager will work mainly with children and families in the Rockland/Camden area and one with work primarily with adults while also seeing several children in Knox and Waldo counties.

Our case managers work in the Behavioral Health Home program and work with children and adults.

The right candidate will truly love helping people and will have exceptional time management skills.
This position will start with almost a full time case load and be benefit eligible within a short time.

Pay is determined by case load and varies.

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
• Handle case assignments, write treatment plans and progress notes, review case progress and determine case closure
• Help clients achieve wellness and autonomy
• Facilitate multiple care aspects (case coordination, information sharing, etc.)
• Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
• Develop effective working relations with team members throughout the entire case management process
• Meet with your clients regularly in their home and local communities
• Link clients and families with local resources, agencies and providers

• Bachelor's in human services or related field
• MHRT-C required
• Minimum of 1 year of case management
