The CARE Director will develop, implement, and oversee advocacy, care coordination, and resource empowerment services within Omni’s holistic behavioral health model. This is a leadership role responsible for designing workflows, supervising staff, coordinating services, and collaborating with clinical, medication management, and community partners to ensure accessible, high quality, and dignified care.

This role includes both strategic leadership and hands on oversight and may be provided through a combination of telehealth and in person services.

Key Responsibilities

  • Develop policies, workflows, and quality standards for advocacy and care coordination
  • Supervise, mentor, and support CARE team members
  • Coordinate services across clinical, medical, and community systems

  • Oversee documentation, compliance, and outcome tracking
  • Support program development, sustainability, and growth
Qualifications
  • Experience in behavioral health, social services, or community-based care

  • Familiarity with Trauma Integrative™ and whole person models of care
  • Strong organizational, communication, and relational skills
  • Bachelor’s or Master’s degree in Social Work, Counseling, Psychology, Public Health, or a related field
  • Licensure or certification in a relevant discipline preferred
  • Supervisory experience strongly preferred
Compensation

This is a high reimbursement-based role with opportunities for growth as the CARE division is established. Compensation may include reimbursement for direct services, administrative and leadership activities, and supervision, with the potential to transition into a salaried leadership position as the program expands.

Why CARE at Omni
  • The CARE division reflects Omni’s commitment to building connection and collaboration for collective healing and meaningful change by supporting individuals and families through advocacy, coordination, and access to community resources.
