Job Summary — PRIMARY
PURPOSE:
Provides professional practical nursing services, direct patient care, and program services under the supervision of a registered nurse to senior behavioral health patients in a manner consistent with the philosophy, standards, and objectives of Catholic Care Center and the Senior Behavioral Health Program. Works collaboratively with the health care team to maintain a safe and therapeutic environment and provides compassionate, person-centered care to patients consistent with role scope, regulatory standards, and practice expectations.
Demonstrates competency to provide age-appropriate care to older adults and geriatric patients (generally 55 years and older).
- Demonstrates performance of tasks necessary to maintain a safe and therapeutic milieu and provide optimum patient care.
- Provides direct patient care and assistance as needed to support patients with ADLs, including dressing, bathing, eating, feeding, and other tasks to ensure quality care.
- Leads and/or assists with program-related activities and groups that reinforce daily goals, learned skills, and cognitive stimulation aligned with role scope and training.
- Uses de-escalation techniques and supports a trauma-informed, person-centered approach toward patients.
- Performs patient rounds and safety checks; reports safety issues to the registered nurse promptly during the shift.
- Performs and documents vital signs as assigned.
- Demonstrates appropriate use of patient care equipment needed to perform role tasks.
- Follows unit safety practices and procedures, including hand hygiene, PPE use, patient assistive devices, falls prevention, and contraband prevention.
- Under direction of a registered nurse, performs the following: provides and receives shift reports, monitors patient condition according to standards, and guides/assists non-licensed staff to complete tasks to care standards.
- Obtains, receives, transcribes, and carries out physicians’ orders as directed.
- Performs all aspects of medication administration per physician’s orders.
- Completes accurate documentation in the patient record to reflect care provided and completes discharge documentation as assigned.
- Obtains and responds to vital signs and other clinical data as indicated; provides patient education and documents.
- Participates in treatment team meetings as assigned; ensures tasks align with LPN scope and standards; demonstrates nursing competence via the nursing process to optimize patient outcomes.
Education — Graduate of an Accredited Practical Nurse Program. Licensed to Practice Nursing in The State of Kansas.
Experience — Minimum of one year in hospital care setting or senior living environment; one or more years in an acute senior behavioral health hospital program preferred. Basic computer knowledge (Word/Excel) and skills in rehab-related or EMR systems desirable. Basic knowledge of EMR preferred.
Special Qualifications — CPI de-escalation program certification preferred;
Trauma-informed care training preferred.
- Basic certifications: BLS/CPR and other certifications as determined by specialty and clinical setting.
- Able to meet the physical requirements of the position.
Physical Requirements:
Walking, standing, and/or stooping for long periods. Mild to moderate physical labor. Exposure to blood, chemicals, and other bodily fluids. Frequent lifting of 50 lbs. or more. Other physical demands associated with direct patient care for older adults and individuals with physical and cognitive limitations. Maintain a safe and clean working environment, adhere to infection control policies, dispose of biohazardous materials per policy, and preserve confidentiality.

May be exposed to unpredictable patient behaviors in an acute behavioral health care setting.
