Social Worker Senior Care Centre

Responsibilities

• Manage and follow up all-new e-referrals from AIC.
• Client initial assessment for ADLs, functional, environmental psycho-social and health needs.
• Monitors and verifies services each client receives and evaluates the effectiveness of each intervention.
• Manage cases and provide family counselling services.
• Conduct interviews to determine client eligibility.
• Supervise all claims eg. Subvention & SMF.
• Supervise application of Senior Mobility Funds.
• Seek alternative funds to support the clients' financial assistance.

• Bachelor’s Degree in social work
• Minimum 2 years of relevant experience in social service/healthcare sector
• Team player with good interpersonal, organisational and communication skills
• Familiarity with the system and schemes such as SMF and Medifund will be an advantage
