  40 hours guaranteed per week
• Start Date: ASAP
• Pay: $35 per hour

• Provide direct patient care including administering medications, treatments, and documenting care per facility policies
• Perform assessments and report changes in resident conditions to RN or management
• Assist with admission, transfer, and discharge procedures for residents
• Ensure compliance with safety, infection control, and quality standards
• Collaborate with interdisciplinary team to develop and implement care plans
• Provide emotional support and education to residents and families

• Minimum 2 years of experience as a Licensed Practical Nurse in Long-Term Care (LTC) settings
• Active NH LPN License or compact license
• BLS (Basic Life Support) Certification
• Must live within 70 miles of Bedford, NH and be able to reliably commute to the facility

• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of EMR/EHR charting systems
• Ability to stand, walk, bend, and lift up to 50 lbs
• Familiarity with medication administration protocols
• Ability to pass background check, drug screening, and health clearance
• TB test, physical exam, and up-to-date immunizations may be required
