Service Coordinator/Case Manager
Beckley, WV
Regular, Full Time, Non-Exempt
$16.00/hour – Open Until Filled
Qualifications- H.S. Diploma/GED.
- Must have thorough knowledge of community resources to be able to assist clients.
- Ability to complete tasks with little direct supervision.
- Excellent written and oral communication skills.
- Skills and knowledge required to maintain a professional office environment.
- Ability to maintain thorough and organized records.
- Current valid driver’s license with safe driving record.
- Must pass drug screen and background check.
- Bachelor’s degree preferred
- Experience in social services area preferred
- Working with the homeless population or those at risk of homelessness.
- This position will work with veterans through the Supportive Services for Veterans Families (SSVF) program.
- The SSVF program is designed to lift very low-income Veteran Families out of homelessness and into stable permanent housing and provide comprehensive assistance in the areas of case management, outreach, temporary financial assistance and referrals to help in obtaining VA benefits.
- The SSVF Service Coordinator/Case Manager will assist clients in accessing community resources in a respectful, non-judgmental manner.
- Carry out duties and responsibilities with limited supervision.
- Make decisions and establish work priorities based on needs of executive offices within the scope of policies and procedures.
- Verify client is a Veteran or a member of a family in which the head of household or the spouse is a Veteran. Conduct thorough client intake, assessment and SSVF Homeless Prevention Screening Criteria to determine eligibility and degree of need.
- Assist SSVF participant in accessing public benefits, daily living service, housing counseling and home search.
- Maintain thorough records of client interaction using master client database, including updating records in a timely manner.
- Provide outreach to local community resources and explore partnership opportunities to best serve mutual clients and prevent duplication of services.
- Collaborate with other agency’s programs by taking applications and making referrals as relevant on case-by-case basis.
- Provide strength-based case management services and offer an atmosphere where clients in crisis can receive individualized assistance with employment, transportation, medical care, childcare, parenting and emergency assistance as needed.
- Assist program director with the preparation of reports as needed to fulfill the requirements of the funding source.
- Attendance at various meetings and training as requested, occasional overnight travel.
- Other duties as assigned by director.
- This position will support the counties of Raleigh, Mercer, Fayette, Summers, Nicholas, and Monroe.
