Job Summary

Our nurse and therapist led Intermediate Care Unit offers a rewarding setting, focused on rehabilitation and reablement, with six Specialist Palliative Care inpatient beds. We use a team nursing and therapy approach, delivering person‑centred care, with support workers working directly alongside qualified nurses or therapists.
This Band 3 role is ideal for experienced support staff. Less experienced candidates can start as Band 2 Rehabilitation Support Worker or Healthcare Assistant, with a clear development pathway to progress to Band 3 through training and supervised practice.
We’re looking for committed, compassionate individuals who want to make a meaningful impact every day. This is a 24/7 service and requires availability for nights, weekends, and bank holidays. Key Responsibilities- Contribute to, support, and promote ECCH’s strategic direction, values, and culture in relation to proactive and reactive services.
- Communicate effectively with patients, relatives and carers, respecting patient confidentiality, privacy and diverse cultural backgrounds.
- Participate in one‑to‑one meetings / clinical supervision within the team as required.
- Participate in the learning environment.
- Contribute to the clinical governance agenda through participation in clinical audit when required.
- Contribute to an environment conducive to effective working, respecting, and supporting the delivery of high‑quality clinical services.
- Contribute to change and innovation within the team.
- Work as part of the multidisciplinary team to deliver delegated proactive interventions including reablement and conducting therapy exercises.
- Support therapy staff with access / discharge home visits.
- Support therapy staff to deliver activities to aid functional ability (group exercises, games, puzzles to improve manual dexterity and memory).
- Work in partnership with senior members of the team and ensure the care provided is of high quality.
- Work with patients towards self‑care and independence.
- Support families and carers, being aware of the effects of illness on the family.
- Enable individuals and families to express their feelings and preferences and take appropriate action.
- Provide supportive care to patients approaching end of life.
- Promote effective communication between patients, families and carers, including participation in all decision‑making related to their care.
- Follow an agreed care plan offering a range of clinical activities to ensure best practice is delivered.
- Undertake personal care activities of daily living with patients.
- Support and deliver aspects of activities of daily living (management of dietary and fluid intake, toenail care, continence management, toileting programmes, use of sheaths, catheters and continence pads).
- Take, record and report (if outside normal parameters) blood pressure, respiration, oxygen saturation and pulse rate using NEWS
2. - Undertake observations of skin assessments, continence and nutrition, and report back to senior colleagues.
- Undertake urinalysis, including collecting urine sample as directed, reporting any concerns regarding urination.
- Participate in simple wound care, skin tear pathway, venepuncture, or collection of specimens.
- Participate in health education / training for patients, families and carers.
- Engage in personal development; maintain competence, knowledge and skills commensurate with the role.
- Be accountable for practice, taking every reasonable opportunity to sustain and improve knowledge and competence.
- Provide information as required to complete outcome measures on admission and discharge.
- Maintain legible, accurate and contemporaneous patient records in accordance with ECCH policy.
- Report any changes in patient’s condition to a registered health care professional.
