SUMMARYOF DUTIES
This position will match the needs of Aging and People with Disabilities in the community with current federal, state and local programs or other resources that are available to meet those needs. This is done through case management and by screening applicants for long term community-based care, in-home care and nursing homes. This case manager position will coordinate the initial intake process to determine eligibility for long term care and services.
This individual will also act as an advocate for clients with community partners to manage and coordinate care.
- A Bachelor's degree in a Behavioral Science, Social Science, or a closely related field.
- A Bachelor's degree in any field and one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
- An Associate's degree in a Behavioral Science, Social Science or a closely related field and two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
- Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
- Experience determining eligibility and appropriate levels of service as a Case Manager
- Experience managing conflicting deadlines and priorities while adhering to policies and procedures
- Experience preparing and maintaining written narrative documentation of actions taken
- Experience interviewing to obtain information to evaluate needs and next steps in the intake process as a Case Manager

This position is a Bilingual (English/Spanish) preferred position. This language skills are not required but preferred. This position may receive a 5% differential pay based on passing the required language test.

This position will primarily work from the hospital with some days in the general office environment. You will visit clients, hospitals, nursing homes, APD offices and other providers. A current driver’s license, and an acceptable driving record is required.
