Epic Healthcare - NOW HIRING: MDS Nurse
Are you an experienced MDS Nurse looking to make a meaningful impact? Our compassionate and dedicated team is seeking a motivated and detail-oriented MDS Nurse to lead the assessment and care planning process for our residents.
What You’ll Do:- Coordinate and complete the MDS assessments and care plans in compliance with state and federal guidelines
- Collaborate with interdisciplinary team members to ensure accurate documentation and individualized care
- Monitor quality indicators and support initiatives for improving resident outcomes
- Ensure timely submission of MDS assessments to maximize appropriate reimbursement
- Active RN license in the state of Ohio
- MDS experience in a long-term care or skilled nursing setting (Required)
- Knowledge of RAI process and PDPM guidelines
- Strong organizational and communication skills
- Ability to work independently and as part of a team
