As the Director of Health & Wellness
, you’ll oversee the delivery of exceptional nursing services in our residential care facility. You’ll provide leadership, training, and support to our care team, ensuring every resident receives personalized attention, appropriate medical care, and respect every step of the way. From monitoring health status and collaborating with physicians to training staff and supporting families, you’ll help residents live with purpose and comfort in a setting that feels like home.
Location: 4175 Indian Ripple Rd, Beavercreek, OH 45440
Status: Full-time, salary
Schedule: Primarily daytime hours with flexibility as needed
Pay: Starting at $81,000.00/year, based on experience

- You’re a licensed RN or LPN in the state of Ohio with a deep commitment to resident-centered care.
- You bring at least 3 years of experience in a senior living or long-term care setting.
- You’re skilled in dementia care and thrive in dynamic environments where every day brings new opportunities to lead and serve.
- You’ve got strong critical thinking and decision-making skills, with the ability to coach, support, and inspire a care team.
- You’re a pro at documentation and communication—especially with electronic health record systems like Point Click Care (PCC).
- You are comfortable being on call or adjusting your schedule as resident needs arise.

Responsibilities- Plans, organizes, develops and directs the overall operation of the nursing department to ensure the highest degree of quality resident care in accordance with all applicable laws and regulations
- Determines if resident is appropriate for admission and remains in our level of care
- Manages the health care of all residents, including the dissemination of information to families and staff
- Supervise staff who have been delegated and/or assigned nursing tasks
- Educates and trains staff, develops ongoing training materials for care staff and focuses on team development and appropriate delegation of tasks within the department
- Review of all incident reports and follow up as necessary
- Assures that TB and Hepatitis B tests are coordinated and completed in a timely manner for staff and residents
- Notifies the AVP of Quality and Risk Management when a resident has a change in behavior exhibited by aggression, combativeness in dementia, pattern of frequent falls and change of condition resulting in a move out
- Assists residents with their daily medications, treatments and resident care as assigned and within the scope of practice, policy and procedures and state regulations
- Responsible for maintaining a current accounting of all medications residents are taking and the documentation required for that process
- Responsible for reporting all change in status of residents as it relates to assisting with medications
- Communicates with physicians and family members as appropriate
- Attends daily stand-up meetings; reporting any resident issues or concerns as documented in the electronic record.
