Overview
Beaver Dam Health Care Center is a skilled nursing home facility that provides residents with care and a positive, compassionate environment. We are currently recruiting for an Occupational Therapist to join our team.

Responsibilities- Evaluate and document evaluations of referred residents in accordance with facility policies and procedures.
- Provide appropriate treatment to help residents maintain or attain the highest level of function possible within their physical abilities.
- Recommend and/or provide appropriate treatment, preventative measures and adaptive equipment to prevent decline in residents’ function.
- Provide appropriate resident or caregiver education to help residents maintain or attain the highest level of function possible within their physical abilities.
- Maintain timely and accurate resident reports as required by federal, state and local regulatory agencies and third-party payers.
- Make recommendations to administration for adaptive equipment and preventative measures to prevent decline in residents’ function.
- Observe infection control procedures.
- Be responsible for the safety of residents under their supervision.
- Assume duties related to completion of Minimum Data Set (MDS) assessments as assigned.
- Complete documentation related to evaluations, treatment plans, MDS assessments, resident care plans and progress notes in a timely manner.
- Participate in resident care conferences, interdisciplinary care conferences and rehabilitation conferences as scheduled.
- Participate in implementation of programs to improve resident care related to Quality Indicators.
- Assume Quality Assurance duties as assigned.
- Consistently work cooperatively with residents, residents' representatives, facility staff, physicians, consultants and ancillary service providers.
- Observe all facility policies and procedures, safety policies and the employee handbook guidelines.
- Follow facility Residents' Rights policies and corporate compliance with Code of Conduct and policies and procedures.
- Attend Corporate Compliance Program in servicing as required.
- Comply with facility policies and procedures against discrimination and sexual harassment, and protect the privacy of resident Protected Health Information.
- All other duties as assigned.
- Communication/Language Skills — Excellent verbal and written communication and organizational skills; ability to read, analyze, and interpret common medical information; ability to effectively communicate with residents, families and other healthcare professionals.
- Reasoning Ability — Ability to define problems, collect data, establish facts, and draw conclusions.
- Computer Skills — Familiarity with software such as Word and Excel.
- MS in Occupational Therapy and NBCOT Certified.
