Full-Time | Skilled Nursing Facility

Beaumont Health Care Center is seeking a strong, confident Charge Nurse (LVN or RN) to lead our team. This is a key leadership role within our skilled nursing facility, ideal for a nurse who thrives in a fast-paced environment, values teamwork, and takes pride in delivering high-quality, resident-centered care.

$5,000 Sign-On Bonus for Full-Time Hires

• Lead and coordinate the 2:00 PM–10:00 PM shift operations
• Conduct shift huddles, manage handoffs, and adjust assignments as needed
• Support staffing coverage and ensure smooth workflow

• Assess changes in resident condition and initiate appropriate interventions
• Administer medications and treatments per physician's orders
• Complete admissions and readmissions, obtain orders, and notify families
• Step in to cover a medication cart when necessary

• Supervise and support CNAs and LVNs on shift
• Provide real-time coaching and reinforce policies and best practices
• Promote accountability, safety, and strong clinical standards

• Ensure accurate and timely EMR documentation (MARs, TARs, progress notes, incidents)
• Support infection prevention, falls management, and QAPI initiatives
• Maintain survey readiness and regulatory compliance

• Communicate effectively with the DON, IDT, providers, pharmacy, therapy, and families
• Escalate concerns appropriately and promptly

• Active Texas LVN or RN license in good standing required
• Skilled Nursing / Long-Term Care experience required
• Prior Charge Nurse or leadership experience is preferred
• Current BLS/CPR certification
• Strong delegation skills and clinical judgment
• EMR experience (Point Click Care  preferred)
• Ability to work 2:00 PM–10:00 PM with every-other-weekend and holiday rotation
