Bella Terra is part of the

** Key Responsibilities
*** Develop and implement individualized
** plans of care
** in partnership with patients, caregivers, and interdisciplinary teams.
* Provide
** preventative and rehabilitative nursing procedures
** as ordered by the physician.
* Deliver
** patient, caregiver, and family education and counseling**.
* Prepare and maintain accurate clinical notes.
* Communicate effectively with physicians and healthcare team members regarding patient needs and changes in condition.
* Identify discharge planning needs and implement prior to patient discharge.
* Supervise
** Licensed Practical/Vocational Nurses (LPN/LVN)
** and
** Home Health Aides (HHA/CNA)
** as directed.
* Participate in
** on-call duties**, quality improvement programs, and in-service training.
** Qualifications**
* ** Active California RN License
*** Minimum
** 2 years of nursing experience** (home health preferred)
* Current
** CPR certification
*** Reliable transportation and valid driver’s license with insurance
* Strong observation, problem-solving, and communication skills
* Ability to work independently and prioritize workload
* Physical ability to lift, transfer patients, and perform nursing duties
