Position: Psychiatric Registered Nurse (ACT Team – Community & Care Coordination)

📍 Lake County, IL (Beach Park / Libertyville)

At AOR, we provide compassionate, community-based mental health services that empower individuals to take ownership of their recovery. We’re hiring a Psychiatric Registered Nurse to join our Assertive Community Treatment (ACT) team in a unique hybrid role blending psychiatric nursing, care coordination, and community-based case management
.

Approximately 75% of care is delivered in client homes and community settings rather than in a hospital or clinic. You’ll support individuals with serious mental illness and co-occurring disorders through assessment, medication education, crisis intervention, and ongoing care coordination—helping clients stay stable, out of crisis, and moving forward in recovery.

What You’ll Do
  • Conduct health and mental status assessments (in-home, community, or telehealth)
  • Provide medication education, monitor adherence and side effects
  • Coordinate care with psychiatry, primary care, and community providers
  • Participate in treatment planning, recovery goal setting, and case coordination
  • Transport clients in your personal vehicle to medical appointments and community-based locations as needed
  • Respond to crises using trauma-informed clinical judgment; participate in shared on-call phone rotation
  • Support coping skills and independent living skill development
  • Provide clinical guidance to LPNs and collaborate closely with the interdisciplinary ACT team
  • Document services accurately in the client management system
What We’re Looking For
  • Active Illinois RN license (or eligible within 60 days)
  • Associate Degree in Nursing required (BSN or MSN strongly preferred)
  • BLS or ACLS certification
  • 0–2 years of mental health experience preferred (early-career nurses welcome)
  • Valid IL driver’s license, personal vehicle, and auto insurance (100k / 300k)
