Summary
The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist helps determine a treatment plan, performs interventions to improve and enhance the patient's well-being, and evaluates the patient's progress.
Pay Range: $110,000 - $120,000, depending on experience.
Responsibilities
- Evaluate the patient's functional status, all relevant body systems for CMS documentation, and physical therapy needs. Consult with the physician to develop the therapy plan of care.
- Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
- Observe and report the patient's response to treatment and any changes in condition to the nurse supervisor and/or physician.
- Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using accepted standards and evidence-based techniques.
- Instruct and inform the healthcare team, patients, and families about the plan of care, functional limitations, and progress toward goals.
- Visit patients within the service area and nearby agencies as needed.
- Complete a progress note for each visit and submit it per policy.
- Complete patient evaluations and develop the PT plan of care within state guidelines. Report outcomes, goals, and projected care frequency within 24 hours.
- Participate in staff conferences and committees as requested.
- Supervise home health aides and PTAs as allowed by policy and state regulation.
- Maintain schedule flexibility, participate in weekend rotations, and be available on observed holidays.
- Prepare discharge summaries and condition reports at least every 60 days.
- Provide in-service education as needed.
- Follow LHC Therapy Standards of Practice and maintain up-to-date clinical knowledge and licensure.
- Perform any other duties as assigned.
Education and Experience
Licensure Requirements
- Current Physical Therapy license in NJ.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle.
