Therapist- Youth ACT

Overview

Hope For Youth, Inc is hiring a Therapist for our Youth Assertive Community Treatment (ACT) Program in Bay Shore, NY. The ACT Program is licensed by the New York State Office of Mental Health and serves children ages 10 to 21 who are at risk of intense or residential treatment. The program delivers intensive, trauma‑informed community support to keep youth safely in their homes whenever possible.

• Serve clients in home, community, and office settings using person‑centered family, individual, and group therapy.
• Maintain documentation and reports in the electronic health record and participate in daily interdisciplinary team meetings and family meetings.
• Conduct comprehensive clinical assessments, develop treatment plans, and apply evidence‑based interventions tailored to each client’s needs.
• Provide crisis management and cover rotations outside normal business hours.
• Coordinate with DSS, schools, and other child and youth collateral contacts.
• Attend trainings, professional development, and team meetings as assigned.

60% Direct client therapeutic contact (including transportation)
20% Case documentation and reports
20% Staff/team meetings, supervision, and training

• LMSW, LCSW, LMHC, or LMFT (limited permits acceptable).
• Minimum 1 year of experience working with children/adolescents and families preferred.
• Proficiency with Microsoft Office Suite and experience with an electronic health record system.
• Valid New York State driver’s license in good standing.
• Physical ability to read, write, and stand for prolonged periods; ability to bend, stoop, kneel, and walk for daily access to own vehicle.

• Competitive salary range $65,000 – $68,500 per year.
