Position: Program Manager - AFC GROUP HOME MANAGER - Bay Valley

Overview

The Program Manager is a key associate at Hope Network whose primary responsibilities include the oversight and management of one or more Hope Network Residential homes. The Program Manager is responsible to hire, train, coach and mentor direct care workforce members so that person-served receive care consistent with their personal plans and goals. The role ensures facilities and residential services meet applicable licensing rules, accreditation standards, and contract/funder requirements.

The position also aims to meet performance targets set by the leadership team related to service provision, quality assurance, and fiscal viability, and to plan, organize, and direct service provision, protect the rights of persons served, develop and maintain budgets, allocate resources, and assess and document the progress of persons served in a timely manner.

Responsibilities
  • Regular and predictable attendance is an essential requirement of this position.
  • Manage the program in a manner consistent with Hope Network’s mission, policies, and compliance with standards established by Hope Network, our funders, CARF and all licensing and accreditation standards.
  • Be on-call twenty-four hours each day, seven days a week and able to respond when emergencies arise.
  • Interview, orientate, train and recommend applicants for hire.
  • Develop and retain workforce members through performance reviews, discipline, development activities, and formal and informal training in accordance with Hope Network’s Workforce member Manual.
  • Respond to workforce member needs and questions; be a role model for good morale and promote positive work relationships.
  • Provide leadership to residential workforce members on clinical, workforce, and programmatic issues.
  • Hold regular workforce member meetings and utilize a communication log to ensure appropriate flow of information.
  • Work toward maintaining the highest level of care and quality for person-served; implement established procedures to address routine issues as well as crisis management.
  • Review person-served plans regularly and provide on-the-job coaching to workforce members implementing the plan.
  • Attend and advocate for person-served at PCP meetings.
  • Ensure residential teams are properly trained and that opportunities are available for ongoing development through training and workforce member meetings.
  • Review all incident reports, sign off and forward to agency designee; review IRs and corrective action with workforce members and/or provide training to prevent re-occurrence.
  • Provide leadership, support and direction to workforce members through: communicating essential information, soliciting feedback, and following up on concerns/questions.
  • Chair monthly workforce member meetings in-house and participate in monthly management meetings and other necessary meetings.
  • Promote and encourage workforce member participation and attendance for corporate functions.
  • Participate and work as a backup to support during times of crisis or lack of workforce members.
  • Maintain staffing levels in accordance with program needs and understand treatment plans to ensure a staffing team meets needs.
  • Monitor and validate workforce member time clock/timesheets and track hours to ensure sufficient hours and minimize overtime.
  • Post schedules a minimum of two weeks in advance.
  • Develop and maintain positive, working relationships with external funders, licensing and accreditation bodies, parents and guardians.
  • Submit all paperwork/records pertaining to homes correctly and in a timely manner, including budgets, invoices, licensing and accreditation standards, recipient rights and corrective actions.
  • Safety: train, promote and monitor safety programs; conduct fire, safety, and tornado drills; participate in safety meetings; implement emergency evacuation procedures.
  • Fully responsible for all aspects of daily operations and maintenance of functions including, but not limited to, vehicle and home maintenance; ensure safe transportation services and documentation, and efficient use of workforce member and vehicle resources.
  • Communicate needs and concerns as a liaison between Hope Network and contracting agencies.
