Physical Therapist - $10,000.00 Sign On Bonus

Position Summary:

The physical therapist will accurately assess, differentiate diagnosis, and identify deficits in a patient’s ability to move and perform functional activities in daily life. They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability and work with the individual to prevent loss of mobility.

• Performs patient evaluations through history, relevant systems review, and specific tests and measures.
• Establishes an individualized plan of care and selects treatment techniques addressing identified impairments.
• Conducts ongoing reassessment of patient response to treatment.
• Initiates and manages the discharge plan from first visit through treatment.
• Supervises and delegates to ancillary staff and students consistent with state laws and policy.
• Completes legible, accurate and clear documentation within established timeframe and accreditation requirements.
• Communicates with patient, family, caregivers, significant others, and health care team utilizing AIDET.
• Assesses educational needs and provides education to meet those needs.

• Graduate of an accredited program of physical therapy.
• Current State Board of Physical Therapy licensure.
• BLS certification.

• One year of physical therapist experience.
• Three years recent clinical expertise in area of concentration.
