Position OverviewThe PRN Case Manager supports patient care coordination from admission through discharge, ensuring continuity of care without disruption. This role serves as an advanced clinical resource for patients, families, and staff while collaborating with interdisciplinary teams to deliver high-quality, cost-effective care across inpatient and Emergency Department settings.
PRN Case Managers provide flexible coverage during variable shifts, weekends, and holidays to support hospital operations and patient needs.
Key ResponsibilitiesPatient Care Coordination
- Manage patient transitions from admission through discharge
- Conduct early assessments to identify patients at risk for post-hospitalization needs
- Develop, implement, and revise transition-of-care plans with care coordination team members
- Round on assigned patients and maintain ongoing communication throughout care
- Coordinate safe, timely, and effective discharge plans
- Identify and arrange post-discharge services including home health, rehabilitation, long-term care, and community resources
- Serve as a liaison between patients, families, physicians, ancillary departments, and community providers
- Ensure appropriate utilization of outpatient and community-based resources
- Monitor patient progress and document according to policy
- Identify readmissions and implement prevention strategies
- Track and trend length-of-stay (LOS) data and address outliers
- Evaluate outcomes related to LOS, readmissions, patient satisfaction, and financial performance
- Translate outcomes into principles of healthcare reimbursement
- Lead and participate in multidisciplinary team rounds
- Build strong working relationships with physicians, nursing teams, and leadership
- Coordinate care using established pathways and plans of care
- Participate in assigned committees and work groups
- Collaborate with providers and multidisciplinary teams to develop and review standard and behavioral care plans
- Assess uninsured and underinsured patients for eligibility in federal, state, and community assistance programs
- Assist patients with applications and collaborate with Patient Financial Counselors
- Serve as a liaison to community programs and maintain relationships with external resources
- Bachelor of Science in Nursing (BSN) required
- Minimum of 3 years of experience in an acute care hospital setting
- Active Michigan RN license in good standing
- Case Management Certification preferred
- Strong critical thinking, decision-making, and problem-solving skills
- Ability to manage multiple priorities in a fast-paced environment
- Excellent verbal and written communication skills
- Ability to explain complex information clearly to patients and families
- Strong negotiation, conflict resolution, and collaboration skills
- Proficient with word processing, spreadsheets, and other relevant software
- Self-directed, adaptable, and able to work independently
- Sustained computer use and concentration (40–70 percent of work time)
- Regular walking, standing, bending, and sitting
- Occasional lifting of light-weight objects
- Ability to function effectively in high-acuity, high-demand situations
- Shift: Variable (weekends and holidays required)
