Position: Advanced Practice Nurse (Nurse Practitioner/Primary Care)

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200
-000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

  • The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
  • The NP provides advanced clinical practice - consultation - and management
  • The primary purpose of the nurse practitioner (NP) is to practice to the full extent of their education - training and certification as a licensed independent practitioner to provide healthcare to individuals - families - and/or groups in a variety of settings to include virtual modalities
  • Primary Care Core Clinical Privileges will guide advanced practice beyond basic education and are renewed every two years
  • The NP has prescriptive authority for a panel of patients or program as indicated by their position
  • Duties include but are not limited to:
    Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
  • Develops and implements an individualized plan to achieve optimal population health and disease management
  • Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
  • Orders and interprets labs - tests - and diagnostic studies
  • Provides timely follow-up on results - including completion of view alerts
  • Performs and records history and physical examinations and records data from clinic visits
  • Enter progress notes in computerized medical record and completes workload documentation in a timely manner
  • Prescribes pharmacologic and non-pharmacologic interventions
  • Initiates referrals and consults to specialty services
  • Provides education to patients and their families
  • Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
  • Consults other specialties as appropriate - according to service agreements

  • NP may have other duties as assigned

    Work Schedule:

    Monday-Friday from 8:00a to 4:30p
  • Hours may be rotated or extended based on the needs of the Medical Center and our Veterans
