Job Description
American Traveler is seeking a Physical Therapist with home health experience and an active Michigan license for a day shift assignment in multiple Michigan counties.
Responsibilities- Home health setting covering Van Buren, Calhoun, and Kalamazoo Counties
- Day shifts, Monday to Friday, 8:00am to 5:00pm
- Full-time, 40 hours per week
- Every other weekend required with a rotation day off during the week
- No on-call or floating required
- Responsibilities include evaluating, planning, and administering physical therapy treatment to improve patient function and independence
- Duties may include patient and family education, interdisciplinary team discussions, and participation in departmental process improvement
- Documentation required in the electronic health record per compliance timelines
- Flu vaccination proof required, or exemption request must be submitted within 48 hours of contract signing
- Orientation includes 8 hours of required, non-billable modules to be completed annually
- Locals accepted at the same rate with no radius restrictions
- Return staff may be eligible after a 6 month waiting period
- Active Michigan PT license required
- Current BLS (AHA) certification required
- Minimum one year of recent home health experience required
- Valid Michigan driver's license, daily access to a car, and proof of insurance required
- References required for consideration
