Therapy Management, Inc. is seeking a PRN Physical Therapist to join our outpatient therapy team at Heritage Assisted Living in Battle Creek, MI. What You’ll Do:

• Provide individualized outpatient physical therapy services to residents in an assisted living setting
• Collaborate with nursing, wellness, and interdisciplinary teams to maximize outcomes
• Maintain clear and timely documentation to ensure continuity of care
• Deliver compassionate, patient-centered treatment in a relaxed clinical environment

Requirements:

• Active Michigan Physical Therapist license
• Outpatient, AL/IL, or SNF experience preferred (new grads welcome)
• Strong interpersonal and organizational skills
• Commitment to high-quality, evidence-based care
