Private Duty Nurse (RN/LPN) – 1:1 Adult Home Care
Job Summary
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low‑stress, relationship‑based setting.
Client‑Specific Case DetailsThis opportunity supports an adult client with a history of a motor vehicle accident (2020) resulting in a C1 ASIA‑C spinal cord injury and traumatic brain injury. The client has undergone C1–C2 stabilization and requires specialized, hands‑on care.
Care needs include:
- Neurogenic bladder management, including intermittent straight catheterization
- Monitoring for and managing frequent urinary tract infections (UTIs)
- Mobility assistance — client is able to ambulate short distances (up to 40 steps) with a walker
- Support with activities of daily living (ADLs) and ongoing safety supervision
Day shifts available: 12 hours per shift, 7 days per week of consistent attendant care.
Responsibilities- Monitor and document patient condition and vital signs
- Assist with daily living activities (ADLs)
- Communicate with families and care teams
- Maintain safe, high‑quality, patient‑centered care
- Full‑time, part‑time, and per diem shifts available
- Day, night, and weekend shifts
