The Medical Social Worker provides comprehensive social work services to patients across all age groups, including infant, pediatric, adolescent, adult, and geriatric populations. This role supports inpatient units and the Emergency Department, delivering direct interventions related to mental health, crisis response, abuse and neglect, grief, substance use, and adjustment to illness.
You will collaborate closely with multidisciplinary teams and community partners to support patient safety, behavior management, and effective discharge planning, ensuring continuity of care and timely transitions to appropriate services.
Key Responsibilities Clinical & Patient Support- Provide crisis intervention and psychosocial assessments for patients and families
- Support patients experiencing mental health crises, suicide risk, violence, abuse, grief, and substance use challenges
- Conduct brief, solution-focused clinical interventions using motivational interviewing
- Advocate for patients and families to ensure ethical, patient-centered care
- Collaborate with physicians, nursing, psychiatry, and community agencies to develop safe and effective discharge plans
- Coordinate transfers to psychiatric facilities, substance use treatment, and outpatient services
- Identify barriers to discharge and facilitate complex care planning when needed
- Respond to acute care call downs and hospital codes, including trauma, stroke, AMI, and Code Blue
- Cover multiple hospital units and prioritize needs based on acuity
- Assist with de-escalation and behavior management to reduce workplace violence
- Collaborate with hospital security and law enforcement when necessary
- Thoroughly document assessments, interventions, and plans of care in the electronic medical record
- Complete required legal documentation for involuntary psychiatric hospitalization and guardianship hearings
- Participate in departmental policy development, quality improvement, and compliance initiatives
- Participate in multidisciplinary rounds and case discussions
- Support orientation and mentoring of new Medical Social Work staff
- Maintain required competencies, attend trainings, and pursue ongoing professional development
- Contribute to a positive, collaborative work environment
- Master’s degree in Social Work
- Licensed Master’s Social Worker (LMSW) or Limited License Master’s Social Worker (LLMSW) in good standing with the State of Michigan
- LMSW preferred
- Licensed Professional Counselor (LPC) accepted for employees hired prior to 7/16/2024
- 1 to 2 years of professional experience in healthcare or mental health preferred
Physical Requirements
- Work is primarily in a clean, temperature-controlled hospital environment
- May be exposed to infectious diseases and emotionally charged situations
- Occasional exposure to threatening behavior or violence
- Requires standing, walking, and occasional lifting up to 10 pounds
- Must be able to maintain focus, communicate effectively, and manage multiple priorities in a fast-paced setting
