Job Description:
⦁ The OSH Registered Nurse (Center Nurse) is responsible for delivering patient-centered, coordinated care in a clinic-based value care environment. This role focuses on chronic disease education (diabetes and cardiovascular disease), telephonic triage, medication management, and care plan coordination. The RN will collaborate with interdisciplinary teams to improve patient outcomes, reduce ED/hospital utilization, and enhance patient engagement.
The MA coordinates scheduling, pre-authorizations, record collection, and testing completion prior to appointments or surgical procedures. The position also serves as a liaison between providers and patients, ensuring clear communication regarding care timelines and treatment planning.

Roles and Responsibilities:
⦁ Provide competent, safe, and evidence-based nursing care in a clinic setting.
⦁ Deliver comprehensive patient education, particularly for diabetes and cardiovascular disease management.
⦁ Conduct telephonic triage and symptom management using standardized protocols.
⦁ Perform medication reconciliation, prescription refills, and prior authorization processing.
⦁ Assess patients both face-to-face and remotely with strong clinical judgment.
⦁ Collaborate with interdisciplinary care teams to implement and monitor care plans across multiple provider panels.
⦁ Support preventative care initiatives and hospital diversion strategies.
⦁ Respond to incoming patient calls in a timely and professional manner.
⦁ Perform point-of-care testing, specimen collection, and phlebotomy as needed.
⦁ Document accurately in EMR systems and maintain strict patient confidentiality.
⦁ Participate in care team meetings and clinic operational discussions.
⦁ Delegate appropriate tasks to clinical team members to optimize workflow.
⦁ Travel up to 50% between Baton Rouge, New Orleans, and Lafayette as required.
Required skills/
Experience:
⦁ Active Louisiana RN License or Compact State License (in good standing).
⦁ 2–5 years of RN experience (3–5 years preferred).
⦁ BLS Certification.
⦁ Prior clinic/outpatient experience preferred.
⦁ Strong EMR navigation and computer skills.
⦁ Ability to independently assess patients and work autonomously.
⦁ Excellent communication and patient education skills.
