Position Summary
The EP / Structural Heart Navigator (RN) serves as a clinical coordinator, patient advocate, and program liaison for patients undergoing electrophysiology and structural heart procedures. This role ensures seamless care coordination across the continuum—from referral and pre‑procedure evaluation through post‑procedure follow‑up—while supporting quality outcomes, regulatory compliance, and registry participation. The Navigator works closely with physicians, advanced practice providers, cath lab staff, inpatient units, and outpatient services to optimize patient experience, safety, and program efficiency.
Key Responsibilities Patient Navigation & Care Coordination- Serve as the primary point of contact for EP and structural heart patients (e.g., TAVR, Watchman, Mitra Clip, ablations, device implants).
- Coordinate pre‑procedure testing, consultations, imaging, and clearances.
- Provide patient and family education regarding procedures, expectations, risks, recovery, and follow‑up care.
- Facilitate multidisciplinary heart team conferences and ensure appropriate documentation and communication.
- Coordinate transitions of care across outpatient, inpatient, procedural, and post‑acute settings.
- Collaborate with physicians, APPs, cath lab/EP staff, anesthesia, and inpatient nursing to ensure procedural readiness.
- Participate in patient selection processes according to clinical guidelines and program criteria.
- Support post‑procedure follow‑up, including clinic visits, phone calls, and symptom surveillance.
- Identify and escalates clinical concerns, complications, or barriers to care.
- Assist with data abstraction and submission for required registries (e.g., EP and Structural Heart registries).
- Ensure accurate documentation of clinical data, outcomes, and quality metrics.
- Monitor compliance with CMS, accreditation, and institutional requirements.
- Participate in quality improvement initiatives and performance review meetings.
- Track program metrics such as volumes, outcomes, readmissions, and patient satisfaction.
- Act as a liaison between patients, providers, and departments to ensure timely communication.
- Educate staff and providers on program protocols, pathways, and updates.
- Support community outreach and referring provider education as needed.
- Current Registered Nurse (RN) license in the state of employment.
- Minimum 3–5 Years Of Cardiovascular Nursing Experience.
- Cath Lab, EP Lab, CVICU, or Structural Heart experience preferred.
- Strong knowledge of cardiovascular disease processes and interventional procedures.
- Excellent organizational, communication, and critical‑thinking skills.
- Bachelor of Science in Nursing (BSN).
- Experience with EP and/or Structural Heart programs.
- Familiarity with cardiovascular registries and data abstraction.
- Experience coordinating multidisciplinary teams.
- Certification such as CCRN, CVRN, RCIS, or equivalent (preferred, not required).
