Overview
JOB PURPOSE OR MISSION:
To improve the quality of life of patients and families by coordinating patient care processes of assessment, planning, implementation and evaluation in collaboration with the patient, family, physician, and appropriate support services. Provides nursing services to Cardiovascular Diagnostic Lab patients; sets-up equipment; assists physicians; and assesses patient during procedures and delivery of care. Job is performed for the age population served, as defined in the department's scope of service.
- Coordinate patient assessment through collection of patient s health data and prioritize data collection based on the patient s immediate condition or needs.
- Perform ongoing and systematic comprehensive physical and psychosocial assessments and involve the patient, significant others and healthcare providers as appropriate.
- Document patient care assessments in the medical record and analyze assessment data to determine nursing diagnoses and expected outcomes.
- Determine nursing diagnoses using assessment data; identify patient outcomes based on the nursing diagnoses; prioritize outcomes based on the patient s condition and needs; document diagnoses and outcomes in the medical record.
- Provide triage assessment on all patients and follow department guidelines and policies; demonstrate proficiency in managing patients across all age groups and those receiving Procedural Sedation.
- Collaborate with disciplines to formulate an individualized plan of care based on patient needs and current nursing practice standards; review and update the plan of care daily or as the patient condition changes; document the plan of care and implement interventions identified in the plan.
- Deliver direct patient care using nursing practice standards and universal precautions; ensure patient rights and timely administration of treatments and medications; delegate tasks within the scope of practice and maintain oversight and accountability.
- Coordinate direct patient care from appropriate support services and provide individualized patient education to patients and families; plan patient discharge with the interdisciplinary team and document all treatments, medications, education and ongoing assessments.
- Perform ongoing reassessment according to department standards or as condition dictates; revise nursing diagnoses, outcomes and plan of care; involve patient and family in evaluation and updates as appropriate; evaluate treatment effectiveness and patient teaching.
- Maintain patient safety during procedures; monitor vital signs and equipment; anticipate physician needs during procedures and circulate as needed.
- Coordinate patient assessment through collection of health data and prioritize data collection based on immediate needs.
- Perform comprehensive assessments and document in the medical record; identify nursing diagnoses and expected outcomes; plan and implement care; coordinate an interdisciplinary plan of care.
- Provide patient education, discharge planning, and safety practices; document all treatments and assessments; ensure accurate charting and communication with the care team.
- Experience Requirements: 2 years ICU or Cath Lab experience preferred.
- Educational Requirements:
Advanced Cardiac Life Support (ACLS) preferred. - Special Skill, License And Knowledge Requirements:
Current Louisiana RN licensure. - HIPAA Requirements:
Maintains knowledge of and adherence to all applicable HIPAA regulations appropriate to Job Position, including but not limited to medical records and patient information in all formats. - Safety Requirements:
Maintains knowledge of and adherence to all applicable safety practices appropriate to Job Position including incident reporting, handling of wastes, PPE, exposure control plans, hand washing, environment of care, patient identification, medication administration and monitoring, and transport of patients.
