This job provides individualized, direct nursing care of assigned patients under the direction of a registered nurse, mid-level provider or physician. Assists in data collection and implementation of the delivery of health care services, and assists other members of the healthcare team in providing the highest quality personalized patient care. Maintains a positive work relationship with the health care team, and exhibits the skills to provide care and service with individuals of all assigned age groups.

EducationRequired – Graduate of an accredited school of practical nursing.
Work ExperienceRequired – 2 years of LPN experience
OR
1 year of LPN experience with 1 year supervisory experience.
PreferredExperience in ambulatory care.
CertificationsRequired – Current licensed practical nursing (LPN) license in state of practice.
Basic Life Support (BLS) from the American Heart Association.
Knowledge, Skills, and Abilities (KSAs)- Proficiency in using computers, software, and web-based applications.
- Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
- Good interpersonal skills.
- Ability to work with and maintain confidential information.
- Ability to work in a fast-paced environment with minimal supervision.
- Ability to work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability) and travel throughout and between facilities.
- Provides safe, effective nursing care to ensure an effective patient visit.
- Answers patient questions; relays critical information and escalates issues.
- Completes appropriate medical/legal documentation.
- Administers medication to patients in a safe manner.
- Maintains professional and technical knowledge and licensure through continuing education and professional development.
- Coordinates day-to-day clinical tasks to ensure efficient operation of the department.
- Performs other related duties as required.

Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical demand requirements are in excess of those for Sedentary Work. Duties performed routinely require exposure to blood, body fluid and tissue. The incumbent works in a patient care area;
