Clinical Team Specialist at Elara Caring

Key Responsibilities- Coordinate all patient care services, including referrals and the development, implementation, and periodic updates of individualized patient care plans.
- Review, approve, and evaluate changes to care plans for clinical appropriateness.
- Conduct regular OASIS reviews for all patients.
- Ensure required visits and documentation are completed timely.
- Review plans of care and paperwork for accuracy and compliance with state and federal requirements.
- Monitor timeliness of documentation uploads to HCHB, maintaining record accuracy, completeness, and compliance with licensing regulations, certification standards, and legal and ethical duties.
- Participate in the organization’s strategic planning and QAPI activities.
- Perform other duties/projects as assigned.

- Associate degree in a nursing‑related field (required)
- At least 2 years of home care experience as a registered nurse in a home‑health or hospice environment
- Current, unrestricted RN license valid in the state of work
- Proficiency in enterprise applications such as Homecare Home Base, Workday, and Microsoft Office
- Occasional travel may be required to support field offices during audits or events
- Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
- Ability to sit, stand, bend, lift, and move intermittently and lift 50‑100 lbs
