OverviewAs the Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care as prescribed by the physician to acutely ill or chronically ill patients in their homes, record clinical and progress notes, and coordinate patient care activities/services to provide the highest quality of care in the geographic service area to which you are assigned. You will work under the direct supervision of a Registered Nurse and/or designated supervisor.
PrimaryResponsibilities
- Provides high‑quality clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health‑care team, following all applicable infection control procedures.
- Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency’s electronic visit verification program (as applicable).
- Actively participates in the agency’s care coordination process including timely reporting patients’ needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, case coordinator, and community case managers (as applicable).
- Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
- Current CPR certification.
- Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
- For Louisiana:
Current LPN licensure in LA without restrictions. - 1+ year of experience working as an LPN.
- Home care experience.
- Ability to work flexible hours as required to meet identified client needs.
- Ability to manage multiple tasks simultaneously.
- Ability to work independently.
- Good communication, writing, and organizational skills.
The hourly pay for this role will range from $20.00 to $35.72 per hour based on full‑time employment.
