The RN/LPN provides patient-centered care within their respective scope of practice, supporting providers and the clinical team in delivering safe, effective, and compassionate care. This role includes performing clinical duties, assisting with patient education, and ensuring high standards of care in alignment with organizational policies and nursing regulations.

  • RNs demonstrate proficiency in the nursing process, contribute to care planning, and participate in evaluation of nursing care.
  • LPNs function under the supervision of RNs, Clinic Managers, and/or Providers, providing technical and clinical support while maintaining compliance with the Louisiana State Board of Practical Nurse Examiners.
Essential Job Functions include, but are not limited to:
  • Satisfies Core Competencies
    • Maintain professional, ethical, and legal standards in all nursing practices.
    • Demonstrate accountability and commitment to patient safety and quality care.
  • Clinical
    • Perform patient assessments, record vital signs, and recognize variances or red flags.
    • Administer medications and treatments under provider orders, adhering to safety protocols.
    • Perform point-of-care and in-house lab tests; obtain and process specimens for outside labs.
    • Assist providers with patient visits, including rooming patients, interviewing, and documenting in the EMR.
    • Provide clinical triage and coordinate patient flow.
    • Support infection control and patient safety practices in all clinical areas.
  • Patient Relations & Education
    • Greet and assist patients, responding to needs in a timely and professional manner.
    • Act as a patient advocate, assisting with navigation of healthcare services.
    • Provide patient education on preventive health topics, treatment adherence, and general health maintenance (e.g., tobacco cessation, immunizations, STIs, nutrition, medication compliance).
    • Educate pediatric patients and families on child safety, developmental milestones, and wellness topics (as applicable).
  • Maintain accurate and complete patient charts and records.
  • Facilitate scheduling, referrals, prior authorizations, prescription refills, and follow-up appointments.
  • Participate in team huddles, staff meetings, and interdisciplinary coordination.
  • Safety/Infection Control
    • Strictly adheres to Universal Precautions.
    • Performs aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.
    • Follows job safety requirement. Reports and documents accidents or incidents within 24 hours of occurrence to the immediate supervisor.
    • Organizes exam rooms, including general cleaning, upkeep, and re-stocking of all supplies.
  • Workforce Development
    • Maintain current nursing licensure and certifications.
    • Responsible for maintaining up-to-date knowledge, skills, and abilities.
    • Attends and participates in in-service, monthly staff meetings, continuing education offerings, community training events, conferences, and webinars as appropriate.
Required Education

RN: Bachelor’s or Associate degree in Nursing with current Louisiana RN license.

LPN: High school diploma or equivalent with current Louisiana LPN license.

Skills, Licensure, and Knowledge Requirements

RN: New graduates considered; prior clinical experience preferred.

LPN: 3-5 years of primary care clinical experience preferred (infectious disease/HIV experience may be given preference depending on assignment).

Current Basic Life Support (BLS) Certification required.
