• Position Type:
  Non-exempt, full-time or part-time, hourly role.
• Scope of Role:
  Responsible for providing direct support within a single program or unit, typically supporting 4‑6 individuals in residential settings or up to 10 individuals in periodic programs.
• Key Responsibilities:
  • Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
  • Assist with daily activities:
    Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.

To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.

• Provide direct support or assistance in accordance with individual service or program plans.
• Record accurately and maintain confidentiality.
• Make cost‑effective purchases within the individual’s budget.
• Maintain professional relationships with individuals, families, guardians, and case managers.
• Report abuse, neglect, medical, behavioral incidents, and other incidents as required.
• Respect individuals’ rights and maintain confidentiality.

• Accompany individuals to medical appointments and relay orders to providers.
• Accurately administer or assist with medication, with prompt error reporting.
• Monitor health, document concerns, and communicate with nurses or supervisors as needed.
• Follow procedures for medical equipment and report malfunctions.
• Prepare or assist with meals according to menus and special diets.

• Attend orientation and ongoing training; participate in monthly staff meetings.
• Work scheduled hours and notify supervisor when unable to work.
• Work collaboratively, show courtesy, and communicate openly.
• Maintain a safe workplace, comply with safety policies, and report hazards.

• Transport individuals safely and report incidents.
• Perform housekeeping, keep environment safe, and report hazards.
• Make minor repairs and report maintenance needs.
• Check water temperature during bathing and participate in safety drills.

• Perform other duties and activities as required.
• Use Mobile Punch Authorized (MPA) for timekeeping.

Minimum Knowledge and Skills Required for the Job
• Education and Experience:
  • High school diploma or equivalent preferred.
  • Six months of experience in human services preferred.
  • Must be 18 years of age.
  • Working knowledge of computers.
• Certificates, Licenses, and Registrations:
  • Valid driver’s license in good standing.
  • Car registration and vehicle insurance if providing transportation.
  • All state-required training(s) and certification(s) completed in mandated time frames.
  • Maintain valid driver’s license and meet insurance requirements; approval to drive required.
• Work Environment:
  • Works at the program location and may accompany individuals into the community.
• Physical Requirements:
  • Heavy Work:
    Exerting up to 100 pounds of force occasionally, up to 50 pounds frequently, or up to 20 pounds constantly.
