This job assumes responsibility for the medication management of assigned patients; monitors and evaluates medication therapy for appropriateness based on the patient’s health status, institutional criteria/guidelines, and cost‑efficient measures including medication reconciliation, medication monitoring, and patient education. The role remains integrally involved in the education of Pharmacy and Medical Residents, provides education to health care providers as needed, and supplies pharmacy expert content for order sets, benefit design, formularies, and other pharmacy‑related documents as required.

Education
  • Bachelor of Science in Pharmacy (required)
  • Completion of a PGY1 Pharmacy Practice Residency (3 years related specialty experience accepted in lieu of PGY1 Residency)
  • Doctor of Pharmacy (Pharm

    D) (preferred)
  • Completion of a PGY2 Specialty Pharmacy Residency

Skills and Abilities

(KSAs)
  • Computer skills and dexterity required for data entry and retrieval of patient information. Proficiency with Windows applications and keyboard usage.
  • Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
  • Strong leadership and teaching skills.
Job Duties
  • Provides pharmacokinetic consultation, obtains and uses appropriate laboratory data to evaluate drug medical selection and dosing, and communicates findings to health care practitioners.
  • Evaluates patient response to drug therapy.
  • Documents interventions and conducts outcome studies/evaluations related to clinical activities performed, exhibiting good patient outcomes.
  • Assists with the development of pathways, best practices, and guidelines for the assigned population.
  • Educates staff, students, patients, health care providers and other health care professionals. Submits work for publication once every two years.
  • Mentors residents, students and new staff to meet departmental goals.
  • Responsible for overall patient care beyond the hospital visit, through all transitions of care.
  • Volunteers to complete any work/task during staffing shortages caused by inclement weather, electrical power outages, computer downtime, or other situations.
  • Adapts behavior to the specific patient population, including respect for privacy, appropriate introduction, adapted explanations, permission requests and communication style.
  • Other related duties as required.

Physical and Environmental Demands

Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical demands are in excess of those for Sedentary Work. The job may be rated Light Work when it requires walking or standing, or sitting most of the time with occasional pushing or pulling, or working at a production rate pace that involves constant pushing or pulling of materials even when the weight is negligible.

Mechanical lifting devices (carts, dollies, etc.) or team lifts should be utilized. Must be able to stand for prolonged periods, travel throughout and between facilities, and work with patients who have known or suspected communicable diseases.
