The Neuro Medical Center Rehabilitation Hospital is in need of a Medical Social Worker.
Responsibilities- Complete discharge planning and provide counseling for neurologically complex inpatients.
- You must attend weekly team meetings to address patients needs and provide resources to patients and families.
- Responsibilities also include case management.
- Must be a Licensed Clinical Social Worker (LCSW)
- One year or more experience
- Excellent customer/patient service and communication skills
- Masters degree in social work from an accredited university
- Experience in Case Management
