Overview
The primary purpose of this post is, as part of the management team of the Community Resource Centre, to provide efficient and effective person-centred services for older people, designed to meet assessed needs of service users including those living with dementia, physical and learning disabilities, and sensory impairments, to enable individuals to live as independently as possible in their care setting.
This will involve providing services to encourage service user functional rehabilitation.
Service Delivery

- With the Registered Manager participate in the formulation of detailed team objectives and policies.
- With the Registered Manager ensure the effective and efficient implementation of Council policies and the achievement of the Council's objectives, including financial objectives.
- To contribute to co-operative working across services in accordance with the Council's Vision and Values. To contribute to cross-service initiatives as required.
- To assist in ensuring the Team's services are responsive to community needs and that equal opportunity and health and safety issues are identified and addressed effectively.
- To assist in ensuring effective external and internal working relationships are established and maintained with organisations and agencies relevant to the work of the service.
- To ensure that effective internal working relationships are established and maintained with staff in the Residential Service and their trade union representatives; and between the Residential Service and other departments.
- To ensure effective and accessible communication with staff, service users, the general public and others as appropriate.
b) Functional
Service-user care- To coordinate the provision of holistic care and support of the care floor to meet users' physical and psychological needs and to promote the continuing spiritual, family and social life of the residents both within and outside the home, helping them to maintain social interaction with family, friends and the community in line with their wishes.
- To coordinate the delivery of personal care to individuals, following instructions and recognised training received. This may include, for example, help with:
- Getting up/putting to bed
- Personal hygiene
- Use of toilet/commode
- Catheter care including stoma and sheath care
- Assisting with prostheses (false legs/eyes etc)
- Application of simple dressings
- Foot and hand care
- Basic nursing care during illness and terminal illness
- Health promotion
- Person-centred support for those living with dementia
- To assist the Registered Manager in monitoring the effectiveness of the key worker systems within the home, including the preparation, implementation and review of individual care plans, and attending reviews.
- To liaise with medical and nursing practitioners, medical establishments, social workers and other agencies on all relevant matters, including the admission, discharge, transfer and death of service users.
- To assess the progress of service users and to maximise their independence levels, encouraging them to achieve their full potential.
- To monitor the general health and well-being of service users for any signs of deterioration, so that early intervention can ensure that preventable measures are taken to avoid unnecessary hospital admissions.
- To be familiar with and practice in accordance with the Councils Safeguarding Adults Policy and practice guidelines and always relate any issues or concerns to a line manager.
- To maintain accurate service user records, e.g. Care Plans, as required and in accordance with Council Policy and good practice.
- To promote effective communication and positive relationships, and, where there are communication difficulties, to enable service users to have access to advocates where appropriate.
- To operate and promote an effective and responsive complaints procedure in line with Council policy.
- To ensure the proper control, recording and issuing of medications as prescribed by a medical practitioner and in accordance with council policy. To support those who are able to administer their own medication.
