This would be the first Admiral Nursing service in Somerset and an exciting opportunity to make a profound impact ondementia diagnosis, care, and family support across our local communities-and we are looking for adedicatednursewith awillingness to develop a new service within our Frailty Team & PCN
Main duties of the jobAdmiral Nurses are specialist dementia nurses whotransform the lives of people living with dementia and those who care for this brand-new role, you will not only provide exceptional,relationship-centred support to families with complex needs, but you will alsohelp build, shape, and influence what outstanding dementia care looks likeacross Mendip.

As an Admiral Nurse you will provide specialist one-to-onesupport, guidance, and practical solutions for families, alongside supportingbest practice and providing role modelling across
Mendip
PCN.As the Admiral Nurse Clinical Leadyouwillbebalancingtheclinical caseloadfor the service, alongsidesetting-up,developing and evaluatingtheservice.
You will also influence and lead onthe delivery of work streams that improve the experience of care for peopleliving with dementia, and their families from pre-diagnosis topost-bereavement.
Mendip PCN is a group of 5 practices within Central Mendip, Mendip Country Practice, Coleford, Beckington Family Practice, Oakhill Surgery, Grove House Surgery and Park Medical Practice both in Shepton Mallet. Our PCN team at Highfield House was established in May 2022, and consists of the following teams which work collaboratively together supporting the five practices. Teams are Health Connectors, Pharmacy Team, Care Co-ordinators, Frailty Nursing Team and a home visiting Paramedic Team.

You will become part of the wider network of Admiral Nurses and will have the opportunity to access relevant post graduate courses and developprofessionally, as informed by the Admiral Nurse Competency Framework.
In line with the Admiral Nurse Competency framework at an enhanced specialist level, the post holder will provide:
Specialist bio-psychosocial assessment and support for people with dementia and families with complex needs
Facilitation of education and training for staffin the delivery of evidence-based dementia care
Clinical leadership with policy development and quality improvements in relation to dementia care
Consultancy to other health and social care professionals regarding dementia
Main Duties and Responsibilities:
Clinical
Deliver regular clinical 1to1sessionfor families affected by dementia who meet the referral criteria withindesignated areas/ practices
Provide clinical leadership on the care of people with dementia and their carers/families with complex care needs from peri-diagnosisto post bereavement support, to support reduced reliance on GP appointments
Provide specialist nursing assessment, using the Admiral Nurse Assessment Framework or appropriate tools for carers/families affected by dementia, including positiverisk taking, managing distressed behaviours and risk minimisation strategies
Providea range of bio-psychosocial interventions to promote health and wellbeing forpeople with dementia and their carers/families
Work collaboratively with other professionals to improve the identification and management of co morbidities and frailty for people with dementia and their carers and families
Develop and support care plans for people with dementia and families including positiverisk management
Supportpeople with dementia, carers and families and staff to understand and respond tochanges in behaviour and relationships as a result of dementia
Provide care coordination for a clinical caseload of families with complex needs as identified within triage assessment
Ensurethe safeguarding of vulnerable adults and that all legal requirements relating to Mental Capacity Act 2005, Mental Health Act 1983 (amended 2007), and Deprivation of Liberty Safeguards (2009)[1] are considered and adhered to inrelation to patient care
Actas an advocate for people with dementia and their carers and families to promote their rights and best interests
Participateand lead on case conferences, continuing health care processes and bestinterest meetings where required
