Program Overview

The Bastrop State Hospital Step Down (SHSD) Program serves individuals transitioning from the state hospital to a community based residential program. As a specialty home, this program is designed to intentionally and effectively assist individuals with intellectual developmental disabilities or autism co-occurring with behavioral health needs who are reintegrating into the community while promoting resident autonomy and ability to live independently.

For 6 – 12 months, residents receive person centered recovery planning, relapse prevention, skills training, peer support, medication management and medication education. Support with connection to other community resources, including primary care, dental care, and other outpatient needs, are part of the transition process.

Job Description

The SHSD program began in 2020 as part of an initial pilot program in Texas. The residents transitioning into the SHSD program were previously hospitalized for over one year in the state hospital setting, and provided a new, unique option for care in the community.

The Services Manager leads communication with staff and contractors on strategy for maintaining essential operations, including during emergencies and disasters.
