Job Summary:
This is a professional position responsible for facilitating the transfer of dependency cases from the Department of Children and Families to Heartland for Children’s subcontracted case management organizations and managing the operational oversight needed to ensure children achieve timely permanency.
Essential Functions:Establish and maintain positive working relationships with HFC internal and external customers, Department of Children & Families and the case management organizations
Complete pre-work to assess the readiness of case transfer, determine which providers and services can be implemented timely and to ensure the CMO assigned is prepared and supported in becoming the primary worker on the case.
Review case history to ensure all placement options for the youth has been explored and discussed.
Facilitate case transfer staffings and ensure cases are appropriately transitioned to HFC’s Case Management Organizations
Work collaboratively with DCF and CMO’s to ensure decisions are made with fidelity to the Safety Decision Making Methodology model
Work collaboratively with internal HFC departments and CMO’s to ensure permanency and placement decisions are made to support child safety and is aligned with the mission, vision and values of HFC
Analyze HFC/DCF data and measures to report and/or evaluate agency compliance with outcomes for children in order to target challenge areas requiring technical assistance and oversight
Provide technical assistance and guidance to the case management agencies and other key stakeholders with regard to policy and procedure in determining and securing appropriate services as needed
Identify trends, good practice and barriers and make written recommendations to assigned by Program Manager, Chief and/or designee
Analyze case transfer staffings, trends and removal patterns to provide monthly data
Log and track budget, expenses and staffings as well as being a point of contact for community resources and programs.
Serve as point of contact for all legal case work that is not assigned to case management.
Perform special projects as assigned
Required Qualifications:Bachelor's degree in social services or related field required. Masters degree preferred.
Minimum of three years experience in child protection and/or child welfare REQUIRED. Demonstrated professional application of social work theories and methods in a child protection or child welfare environment.
