Community Physiotherapist - Furness Rapid Response

We are seeking highly motivated, committed and experienced Senior Physiotherapist to support the delivery of our service across South Cumbria.

The role will cover across South Cumbria, based from Furness General Hospital.

You will have the unique opportunity to influence change as we continue to develop our admission avoidance services inclusive of 2 hour urgent community response, virtual ward and rapid response.

Our speciality is admission avoidance of which therapy is a key component of the service delivery. Our aim is to support and treat acutely unwell patients in the community, with a range of escalated health and social care needs. Rapid Response s aims are preventing hospital admission and support early discharge from hospital through a Multi disciplinary approach.

The urgent community 2 hour response target and virtual wards are very much part of our service specification and builds on the visions outlined in the NHS Long Term plan. Our virtual ward beds are run on a Generalist Model which incorporates Acute Medicine, Respiratory and Frailty.

Our team is a multidisciplinary team who collectively integrate skills and share knowledge which includes performing non traditional roles and some cross boundary working. In addition to this we work closely with Primary Care, Community Teams, the Urgent Community Response Hub, District Nurses and many more.

Main duties of the job

As a Senior Band 6 Therapist you will:

  • Deliver high-quality urgent care
  • With your expertise and skill, strengthen the care plans delivered from your senior triage and assessments
  • As part of a multidisciplinary team assess and treat conditions within the patient s own home
  • Plan and implement person centred care especially focussing on frailty syndromes and Respiratory conditions, including chest physio
  • Help to improve outcomes for patients as well as the patient experience both within the service and wider organisation
  • Work collectively to improve system working across acute, community and social care settings
  • Share your knowledge and skill
  • Be able to work autonomously
  • Be open and engaging for some cross boundary working ( training and competency assessment will be offered)
  • Demonstrate an understanding and appreciation for your own role and the different roles within the team and how they work in collaboration
  • Undertake timely assessments
  • Ensure assessments are of a high standard, with planning, implementation and evaluation of care plans accordingly
About us

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our community services for adults are provided in people s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.

Person Specification Qualification/Education
  • Diploma or Honours Degree or equivalent in Physiotherapy approved by the Chartered Society of Physiotherapy/ Health and Care
  • Health and Care Professions Council registered Physiotherapist
  • Evidence of relevant post graduate specialist clinical education
  • Membership of the Chartered Society of Physiotherapy Post graduate training or experience to post graduate Diploma level or equivalent
  • Accredited Practice Placement Educator s qualification (or willingness to attain within 2 years)
Experience
  • Relevant post registration experience as a physiotherapist to achieve knowledge and skills applicable to this post
  • Comprehensive rotational experience as a therapist in the core competencies within Physiotherapy for example: musculo-skeletal, neurology and cardio- respiratory care
  • Experience of working within a team
