- Typical hours are Monday–Friday, 8:30 a.m. – 4:30 p.m.
- Some evening and weekend work will be required to meet program needs
- Travel throughout Simcoe County required

Role SummaryThe Supportive Housing Navigator is part of a multidisciplinary team creating and operating safe, affordable, hopeful communities of housing and shared spaces across a spectrum of affordability, location, size, permanence, and support. This role provides intensive, housing-focused support to people moving from homelessness into permanent housing and ongoing support to help residents remain stably housed. Supportive Housing Navigators support residents to identify goals, overcome barriers, and build hope through housing by offering practical assistance, advocacy, and community connection.
Key Responsibilities Housing Access & Transition- Prepare units for new residents, Including cleaning and basic setup, and working with internal teams to ensure readiness for occupancy
- Facilitate resident intake through Coordinated Access (application, interview, viewing, transition planning)
- Support document readiness (, email, banking)
- Assist with income supports, education, training, and employment
- Discuss housing goals and develop a Housing Plan and budgets
- Support with housing searches, including landlord outreach, applications, and attending viewings
- Participate in interdisciplinary supports with residents and community partners
- Advocate and navigate systems alongside residents
- Create Moving and Housing Sustainability Plans
- Support transition to permanent housing, including referrals and move coordination
- Work with the team to conduct a wrap up interview to seek input regarding our premises, services, and supports
- Support life skills, food security, safety, social inclusion, family reunification and daily routines
- Provide tenant education (rights and responsibilities) and eviction prevention support (financial, unit maintenance, disputes, mental health & well-being)
- Advocate and mediate with landlords, neighbours, and service providers
- Address ongoing housing challenges and barriers
- Work through the Housing Sustainability Process to support residents to stay stably housed
- Connect residents to long-term community supports
- Create opportunities for connection, contribution, and belonging
- Support residents’ mental, physical, and emotional wellbeing
- Assist residents in setting goals actions and timelines and working toward them
- Facilitate access to counselling, health care, and wellness supports and cultural connections
- Develop and maintain individualized Safety Plans
- Complete daily HIFIS and case note documentation
- Maintain accurate records, incident reports, and confidentiality
- Participate in team meetings and organizational initiatives
- Support administrative tasks including inquiries, donations, calendars, and communications and volunteer coordination
- Contribute to continuous improvement of systems and processes
Physical Requirements
This community-based role involves regular movement between residential units and community locations throughout Simcoe County. The position requires the ability to navigate multi-level buildings (including stairs), assist residents during housing transitions, and lift or carry household items weighing up to 23 kg (51 lbs).
Qualifications- Post-secondary degree or diploma and/or professional experience in Social or Human Services or a related field (students considered)
- Relevant certifications (e.g., First Aid, NVCI)
- Experience working with vulnerable populations and complex needs
- Knowledge of Housing First and the non-profit housing sector
- Lived experience in the communities we support is an asset
- Strong communication, empathy, and relationship-building skills
- Experience with Microsoft Office, G Suite, and HIFIS
- Valid G-Class driver’s license and reliable transportation (travel across Simcoe County required)
- Growth mindset, adaptability, and problem-solving skills
- Ability to work independently and as part of a team
Job Type: Full-time
Pay: $55,000.00 per year
