Roles & Responsibilities Coordinate and manage hospice care plans for patients in their homes with compassion and clinical expertise
Conduct regular home visits to assess patient condition and adjust plans of care
Serve as the central point of contact for patients, families, and the interdisciplinary hospice team
Document care thoroughly and accurately in the agency EMR (McKesson or Netsmart experience preferred)
Provide symptom management, education, and emotional support to patients and families
Collaborate with physicians, social workers, spiritual care providers, and other team members to deliver holistic care
Ensure compliance with hospice policies, regulations, and quality standards
Safely travel to rural patients’ homes and manage your caseload efficiently across the service area
Qualifications Current, active RN licensure in the state of Vermont (or compact license where applicable)
Experience as a Hospice RN with hospice case management experience required
Proven experience providing hospice care in a home setting
Prior rural travel experience preferred to navigate dispersed service areas
Familiarity with McKesson and/or Netsmart EMR systems preferred
Ability to work the assigned schedule (Friday–Tuesday) and attend M–F orientation the first week
Reliable transportation for travel to patient homes and the ability to manage a rural caseload
Knowledge & Skills Strong hospice nursing and case management knowledge
Skilled in home health assessments and individualized care planning
Comfortable with symptom and pain management in end-of-life care
Excellent communication, family education, and emotional support skills
Proficient documentation practices and comfort using McKesson or Netsmart EMR
Time management and route planning for rural travel
Team collaboration and interdisciplinary coordination
