In Sync is currently seeking Case Manager for positions in Barre Vermont. The ideal candidate will possess a current Vermont license. This is a 8-Hour Day, 07:00:00-15:00:00, 8.00-5 position in the Care Manager, Hospice RN. Proper State License and have at least 2 years of recent Case Manager experience.
Requirements
- Current Resume
- Nursing License per state
- Current BLS and/or ACLS and/or Specialty Certifications
- 2 current clinical references
- Physical (within 12 months)
- TB skin test (within 12 months)
- Titers - MMR/Hep B/Varicella
- Respiratory Fit Test (within 12 months)
- Current - Tdap/Flu vaccinations
- Must be able to pass background/urine drug screening
