** Exciting Opportunity for a Hospice RN Case Manager!** Join a compassionate and dedicated hospice home care agency seeking an experienced Hospice RN to make a significant impact in patients' lives. If you are passionate about providing quality end-of-life care, we want you on our team!

* Key Responsibilities:
** - Manage hospice cases with a focus on patient and family-centered care - Provide expert hospice care in the comfort of patients’ homes - Collaborate with interdisciplinary teams to create personalized care plans - Conduct assessments, develop care strategies, and monitor patient progress - Educate patients and families on hospice services and resources - Engage in rural travel to provide in-home care

* Qualifications:
** - Minimum 1 year of hospice case management experience - Proven experience delivering hospice care in a home setting - Preferred experience with rural travel - Familiarity with McKesson/Netsmart EMR systems is a plus - Active BLS/BCLS certification required

* Work Schedule:
** - Friday - Tuesday (M-F first week to facilitate orientation) - Open to Saturday - Tuesday availability
** Additional Information:** - No RTO requests preferred - Holiday coverage required
